Description

Healthcare providers have long struggled to overcome the adverse effects caused by the use of tobacco during pregnancy. Some 20 percent of low birth weight births, eight percent of pre-term deliveries, and five percent of all perinatal deaths are linked to smoking during pregnancy, making smoking the most important modifiable cause of poor pregnancy outcome.

In 1994, The Robert Wood Johnson Foundation (RWJF) established Smoke-Free Families (SFF): Innovations to Stop Smoking During and Beyond Pregnancy to oversee the development and evaluation of new, evidence-based interventions to help women quit smoking before, during and after pregnancy. The National Program Office (NPO) based at the University of Alabama at Birmingham funded 11 Phase I research projects in 1995; their work was completed in 1998. Results of these projects appear in the September 2000 (Volume 9, Supplement 3) edition of Tobacco Control.

Planning for Phase II of SFF began in 1998 and included a review of published literature on intervention research conducted since 1985 as well as an analysis of the SFF research project findings. About the same time, the Public Health Service (PHS) and other government and non-government partners re-convened the expert panel that developed the 1996 Smoking Cessation Clinical Practice Guideline No. 18 to conduct an update. As a result of this panel’s work, the US PHS Clinical Practice Guideline: Treating Tobacco Use and Dependence was issued in June 2000. Results of the SFF Phase I research and PHS reviews and meta-analyses confirmed that a brief (five to fifteen minute) counseling intervention, delivered by a trained provider and paired with pregnancy-specific self-help materials, can increase cessation rates among pregnant smokers by 30 to 70 percent.
