The organizational meeting of the SBI was held in Boston, Massachusetts on April 24,1985 by the 6 founding radiologists: Carl D'Orsi, MD; Stephen Feig, MD; Marc Homer, MD; Harold Moskowitz, MD; Myron Moskowitz, MD and Edward Sickles, MD. The objectives of the SBI, as listed in the articles of organization, are to:

Initially, membership in the Society was limited to Fellows and was by invitation, with membership based on contributions to the field of breast imaging. From 1985 to 1990 the Society grew to 75 active fellows. In 1991, a general membership category was established, a natural evolution that provided an opportunity for the Society to increase its influence and effectiveness in scientific, professional and political interactions, and in educational activities. Requirements for general membership are certification by the American Board of Radiology or American Osteopathic Board of Radiology and an active interest in breast imaging. Members who meet specific qualifications may apply to be Fellows of the Society, after one year as general members. There are currently 107 SBI Fellows, including 15 Emeritus Fellows and 6 Honorary Fellows. In the spring of 1995, an Affiliate Member category was established for registered radiologic technologists. Affiliate members now number over 115.

The SBI held its first Postgraduate Course in 1993, at Amelia Island, Florida. Subsequent biennial courses have been held around the country; the May 2005 course held in Vancouver, British Columbia was the first international meeting site.
